Complete this form to request up to five voter registration forms for you and your family or friends.

To obtain forms for conducting voter registration drives, please call (304) 558-6000 or (866) SOS-VOTE to place your request. Information about the location and time period of the drive and other details will be requested.

Please provide the information listed. When you click submit, your request will be sent directly to the Elections Division of the Secretary of State. If you have any problem with the form, please send your request by email to elections@wvsos.com and it will be filled.
