step 4 - Submit Filings Correctly

A.  Complete the Form

The forms specified by law (§46-9-521) are standard forms acceptable in every state which has adopted the uniform act.  These are not the only acceptable forms, but they were designed to reduce error by both filers and filing offices.   To make filing easy, fill out the form on screen using Acrobat Reader (4.0 or higher).  For information about how to download and use the free reader, see Acrobat Reader Help.

1) Choose a form

  • UCC1 - UCC INITIAL FINANCING STATEMENT
    One page, maximum two debtors, one secured party.

  • UCC1 Addendum - UCC INITIAL FINANCING STATEMENT PLUS ADDENDUM
    Two pages, for additional debtors, secured parties or collateral

  • UCC1 Additional Party - UCC1 ADDITIONAL PARTIES
    Detailed information on adding Debtor or Secured  Party names as needed

  • UCC3 - UCC FINANCING STATEMENT AMENDMENT
    One page, for amendments, continuations, assignments, terminations

  • UCC3 Addendum - UCC FINANCING STATEMENT AMENDMENT ADDENDUM
    Two pages, for additional debtors, secured parties or collateral

  • UCC3 Additional Party - UCC3 ADDITIONAL PARTIES
    Detailed information on adding Debtor or Secured  Party names as needed

  • UCC5 Correction -UCC CORRECTION STATEMENT

  • UCC11  Information Request

2) Click in the box and type

3) Print the completed form

  • If two copies are sent, one stamped & dated copy will automatically be returned by mail.

  • If only one copy is sent, a stamped & dated copy will be provided on request.

B.  Review the Completed Form

Missing, illegible or confusing information can result in rejection.  See the filing steps for more detail.  The law specifies errors which require rejection by the filing office (§46-9-516).

1) On an initial statement, check for:

  • The debtor's correct name.

  • The debtor's mailing address.

  • The organization type, organization number (or "None" if there is none), and jurisdiction for a debtor which is an organization.

  • The secured party's name.

  • The secured party's mailing address.

  • The description of collateral.

2) On an amendment, check for:

  • The filing number of the original financing statement which is still in effect.

  • Any new additional debtor's correct name and mailing address.

  • The organization type, organization number (or "None"), and jurisdiction for a new debtor which is an organization.

  • Any new secured party's name and mailing address.

  • Any changes in description of collateral.

3) On a continuation, check for:

  • The filing number of the original financing statement which is still in effect.

  • The expiration date, which must be within the up-coming six months.

4) On an assignment, check for:

  • The filing number of the original financing statement which is still in effect.

  • The name and mailing address for the assignee.

5) On a termination, check for:

  • The filing number of the original financing statement which is still in effect.

C.  Remit the Fee

  • Send $10 for each financing statement or amendment submitted.  Filings without a sufficient fee must be rejected.  Please do not send excess payment -- refunds are very expensive for us!

  • Make checks payable to Secretary of State.  For other payment options, see Ordering and Payment.

D.   Mail to the Secretary of State
